ศูนย์พัฒนาธุรกิจและการดูแลสังคมสูงอายุ (ABCD CENTRE) คณะพาณิชยศาสตร์และการบัญชี มหาวิทยาลัยธรรมศาสตร์ (มธ.) จัดงานสัมมนาพิเศษหัวข้อ “มิติใหม่ของการบริหารจัดการชุมชนเพื่อคุณภาพชีวิตผู้สูงอายุ Community based Care” เมื่อวันที่ 1 ต.ค. 2563 โดยสาระสำคัญเป็นการแลกเปลี่ยนเรียนรู้ร่วมกันถึงระบบดูแลแบบองค์รวมในชุมชน และการดำเนินงานของ Silver Human Resource Centre หรือศูนย์จัดหางานให้ผู้สูงอายุ ของประเทศญี่ปุ่น ซึ่งสามารถนำมาประยุกต์ใช้ในประเทศไทยได้
นายชิเกโอะ อิโต ผู้จัดการฝ่ายนโยบายประกันสุขภาพ แผนกสุขภาพและสวัสดิการ เทศบาลเมืองทามะ กรุงโตเกียว ประเทศญี่ปุ่น เปิดเผยว่า หากย้อนกลับไปเมื่อ 50 ปีก่อน หรือในปี 2513 ประเทศญี่ปุ่นมีประชากรที่อายุเกิน 100 ปี เพียง 339 คน แต่ปัจจุบันกลับมีมากถึงกว่า 8 หมื่นคน และในปี 2568 หรืออีก 5 ปีข้างหน้า ประชากรรุ่นเบบี้บูมเมอร์ของญี่ปุ่นจะมีอายุ 75 ปีขึ้นไป ซึ่งคนเหล่านี้จะมีสมรรถภาพถดถอย มีโอกาสพบภาวะสมองเสื่อม จึงจำเป็นต้องได้รับการดูแลอย่างใกล้ชิด
นายอิโต กล่าวว่า การเพิ่มขึ้นของผู้สูงอายุในประเทศญี่ปุ่นทำให้ระบบการดูแลเดิมไม่สามารถรองรับไหว โรงพยาบาลและสถานดูแลผู้สูงอายุไม่เพียงพอ นั่นเป็นที่มาของการปรับปรุงระบบใหม่ด้วยวิธีการดูแลตามบ้าน ด้วยระบบดูแลแบบองค์รวมในชุมชน หรือ Community-based Integrated Care System ซึ่งมีองค์ประกอบหลัก 5 ประการ ได้แก่ ที่อยู่อาศัย การแพทย์ การดูแล การดูแลเพื่อป้องกัน และช่วยเหลือการดำรงชีวิต
สำหรับ Community based Care ซึ่งเป็นวิธีคิดรูปแบบใหม่นั้น เป็นการดูแลโดยให้ผู้สูงอายุอาศัยอยู่ในชุมชนที่ตัวเองคุ้นเคย พร้อมจัดระบบบริการทางการแพทย์ บริการสร้างเสริมสุขภาพและป้องกันโรคเข้าไปสนับสนุน รวมถึงให้ความช่วยเหลือในการดำรงชีวิต เช่น การซื้อของ การเดินทาง หรือดูแลความปลอดภัยต่างๆ
นายอิโต กล่าวอีกว่า ประเทศญี่ปุ่นยังมีปัญหาสังคมอื่นๆ ที่มากไปกว่าปัญหาผู้สูงอายุ เช่น ปัญหาเด็กเกิดน้อย ปัญหาความสัมพันธ์ของคนในชุมชนลดลง ปัญหาที่เรียกว่า 80:50 คือคนอายุ 80 ปี ได้รับการดูแลโดยคนอายุ 50 ปี ซึ่งหมายถึงผู้สูงอายุต้องมาดูแลกันเอง ปัญหา Double Care คือคนที่ต้องดูแลทั้งลูกและพ่อแม่ที่สูงอายุ รวมถึงปัญหา NEET (Not in Education, Employment or Training) คือลูกหลานไม่ทำงาน

ทั้งนี้ ภายหลังประเทศญี่ปุ่นใช้แนวทาง Community based Care มาเป็นเวลากว่า 10 ปี จึงได้พัฒนาแนวคิดลึกลงไปสู่สังคมด้วยการดูแลแบบอยู่ร่วมกัน โดยตัวอย่างการดำเนินงานของเมืองทามะ ซึ่งมีสัดส่วนประชากรสูงอายุในเมืองเพิ่มขึ้นรวดเร็วที่สุดในญี่ปุ่น ได้มีกิจกรรมเพื่อสาธารณประโยชน์หลากหลายรูปแบบ
อาทิ ระบบการดูแลแบบองค์รวมในชุมชนที่ใช้ประโยชน์จากลักษณะของเมือง ทั้งการประยุกต์ใช้พื้นที่อาคารเพื่อสร้างฐานการพัฒนาชุมชน ใช้พื้นที่สีเขียวสำหรับออกกำลังกาย รวมถึงโครงการป้องกันความอ่อนแอในวัยสูงอายุแต่เนิ่นๆ ทั้งหมดนี้ทำให้ทามะเป็นเมืองที่ใช้เบี้ยประกันดูแลที่ต่ำเมื่อเทียบในระดับประเทศ
นอกจากนี้ ประเทศญี่ปุ่นยังมีการดำเนินงาน Silver Human Resource Centre (SHRC) หรือศูนย์จัดหางานที่จะเป็นตัวกลางในการจับคู่ ส่งเสริมให้ผู้สูงอายุที่สุขภาพแข็งแรงและมีความกระตือรือร้นในการทำงาน มีบทบาททางสังคมด้วยการทำงานเบาๆ ได้ตามที่ตัวเองต้องการ เช่น ครูสอนพิเศษ นักแปลเอกสาร ทำสวน ซ่อมประตูหน้าต่าง ทาสี ตัดหญ้า ดูแลเด็ก พาสุนัขเดินเล่น โดยงานจะมีลักษณะเป็นงานชั่วคราว ระยะสั้น และรับค่าจ้างที่ต่ำกว่าระบบค่าจ้างขั้นต่ำ
สำหรับแนวคิด SHRC ตั้งต้นมาตั้งแต่ปี 1970 โดยมุ่งหวังที่จะให้ผู้ที่เกษียณอายุแล้วมีรายได้เพิ่มเติมจากเงินบำนาญอีกเล็กน้อย แต่ปัจจุบันพบว่าผู้สูงอายุส่วนใหญ่มีสุขภาพแข็งแรงและอยากทำงานเต็มที่เท่ากับคนหนุ่มสาวเพื่อให้ได้รับเงินเดือนมากขึ้น นั่นทำให้สัดส่วนการเข้าร่วมเป็นสมาชิก SHRC ลดลง อย่างไรก็ตามในประเทศไทยที่มีจุดแข็งเรื่องการทำบุญและงานจิตอาสา ภาพของ SHRC ที่จะเกิดขึ้นจึงอาจแตกต่างกันออกไป

ผศ.ดร.ดวงใจ หล่อธนวณิชย์ ผู้อำนวยการ ABCD CENTRE กล่าวว่า หนึ่งในปัญหาการเตรียมความพร้อมเข้าสู่สังคมสูงวัยของประเทศไทยคือเรื่องความมั่นคงทางรายได้ ซึ่งผู้สูงอายุส่วนใหญ่จะมีเงินออมไม่เพียงพอกับเงินขั้นต่ำที่จะสามารถดำรงชีวิตหลังเกษียณได้อย่างเหมาะสม ขณะเดียวกันยังมีผู้สูงอายุอีกส่วนหนึ่งที่ต้องการทำงาน ดังนั้น SHRC คือตัวอย่างหนึ่งที่อาจเป็นทางออก โดยอาจมองควบคู่ไปกับโอกาสทางธุรกิจของประเทศไทย เช่น การท่องเที่ยว อาหาร หรือแม้กระทั่งแนวคิดของ Medical tourism
“เราต้องเปลี่ยน mind set ว่าการทำงานถือเป็นศักดิ์ศรีของผู้สูงอายุ ถึงจะมีบุตรหลานเลี้ยงดูแต่ผู้สูงอายุก็สามารถเลี้ยงดูตัวเองได้ด้วย อย่างไรก็ตามแม้จะเป็นโอกาสทางธุรกิจแต่ก็ไม่ใช่การนำภาคเอกชนเข้ามาแสวงหาผลกำไร ต้องเปลี่ยนมุมมองเป็นรูปแบบธุรกิจเพื่อสังคม ที่ภาครัฐและเอกชนเข้ามาร่วมกันสนับสนุนเพื่อให้เกิดระบบบริการที่ช่วยแก้ไขปัญหาสังคม” ผศ.ดร.ดวงใจ กล่าว
นพ.อรรถพล แก้วสัมฤทธิ์ รองอธิบดีกรมอนามัย กระทรวงสาธารณสุข (สธ.) กล่าวว่า กรมอนามัยได้มีการใช้แนวทาง เส้นทางชีวิตสุขภาพ (Life Course Approach) เป็นทิศทางการดูแลสุขภาพของประชาชนตั้งแต่ในครรภ์ วันเด็ก วัยเรียน วัยรุ่น วัยทำงาน จนถึงวัยสูงอายุ ซึ่งในส่วนของการดูแลผู้สูงอายุ ได้มีการจัดทำ Blue book หรือสมุดบันทึกสุขภาพผู้สูงอายุ เป็นองค์ความรู้ในการดูแลตนเอง และใช้ในการวางแผน สร้างพฤติกรรมสุขภาพที่พึงประสงค์
นพ.อรรถพล กล่าวอีกว่า การดูแลผู้สูงอายุในอนาคตจะเป็นโปรแกรมการดูแลระยะยาว (LTC) โดยการบริหารของ 3C คือ Care Manager, Caregiver และ Care Plan เชื่อมต่อกับการเบิกจ่ายจากสำนักงานหลักประกันสุขภาพแห่งชาติ (สปสช.) เพื่อการดูแลผู้สูงอายุทุกกลุ่มทุกพื้นที่ รวมถึงการมีกฎหมายปฐมภูมิ ที่ทุกคนจะมีหมอประจำตัว 3 คน คือหมอในโรงพยาบาล หมอในชุมชน และหมอในหมู่บ้าน เป็นการดูแลที่ประกอบด้วยทีมสหวิชาชีพในการดูแลสุขภาพ
ด้าน ผศ.ดร.ดารารัตน์ อานันทนะสุวงศ์ ผู้อำนวยการศูนย์วิจัยสังคมสูงอายุ สำนักวิจัย สถาบันบัณฑิตพัฒนบริหารศาสตร์ (NIDA) กล่าวว่า ที่ผ่านมาปัญหาของระบบการดูแลผู้สูงอายุในประเทศไทย คือบทบาทของหน่วยงานที่เกี่ยวข้องยังไม่ชัดเจน ไม่มีการประสานงานกัน ขณะที่ผู้สูงอายุยังไม่ทำความเข้าใจเกี่ยวกับสิทธิของตนที่พึงมี รวมไปถึงโครงสร้างทางทรัพยากรด้านการบริบาล การบริการด้านการแพทย์และสาธารณสุขที่ยังไม่เพียงพอ
ผศ.ดร.ดารารัตน์ กล่าวว่า บทเรียนจากญี่ปุ่นทำให้เห็นว่าไทยจำเป็นต้องมองมิติใหม่ในการบริหารจัดการผู้สูงอายุ โดยบูรณาการระหว่างภาครัฐ แทนที่จะกำหนดนโยบายและการปฏิบัติแยกกันตามหน้าที่ของแต่ละหน่วยงาน พร้อมกับการกระจายอำนาจการดำเนินการให้แก่ท้องถิ่น รวมถึงการมีส่วนร่วมของภาคเอกชน NPOs กลุ่มวิสาหกิจเพื่อสังคม ในการผลิตสินค้าและการบริการเพื่อสังคมสูงวัย (Silver Economy) ตลอดจนการสร้างฐานข้อมูลผู้สูงวัย
